Please read the coverage certificates below prior to submitting a warranty claim:

Warranty Certificate – Overview of the standard SAKAI Warranty.

Extended Warranty – List of additional components covered under the optional extended warranty. For more information on the extended warranty, please contact the warranty team.

Warranty Registration – Transaction Reporting Form (Heavy Equipment) This online form must be submitted upon first retail use to start warranty coverage on a new SAKAI machine. This begins the standard warranty term. No further action is necessary unless a breakdown occurs.

Warranty Registration – Transaction Reporting Form (Light Equipment) – This online form must be submitted upon first retail use to start warranty coverage on a new SAKAI machine. This begins the standard warranty term. No further action is necessary unless a breakdown occurs.

Demo Registration Form – Complete when sending a new machine out on demo. Warranty registration allows for no more than 40 hours per demo registration. Warranty registration is required after machine has accumulated 100 hours.

Warranty Claim Form – Use this excel form start a warranty claim. Make a copy and re-name it as your work order number. Complete the form and email to the warranty group (warranty@sakaiamerica.com). We require pictures of leaks and appreciate pictures of all failures. Attach pictures to the email when sending in your form to SAKAI.

Form Field Instructions
Claim Number Use your Work Order number.
Application Date The date the claim is sent to SAKAI.
Distributor & Address The dealer who did the repair.
Customer & Address Company that owns the machine.
Delivery Date If the dealer Transaction Form was completed, you will have this date at your store. SAKAI America does not establish the warranty start date, dealers do via the Transaction Reporting Form. Get with your Sales department for this info (warranty team will assist if necessary).
Model Use the complete model number.
Serial Number Include prefix and SN.
Hours Hours on machine at time of repair.
Date of Service Date the work began.
Date of Completion Date the work concluded.
Failure Information Use the three C’s: Complaint, Cause & Correction. This information is critical. Explain clearly the complaint, cause, and what was done to make the repair. In most cases the failure can be described in a few sentences. If we need more info, we will respond by email with questions.
Expenses In this section, all warranty expenses are listed. We pay D net + 10% on SAKAI parts, cost on outside invoices.
Parts Subtotal This is the subtotal of your part expenses and is calculated automatically. Do not overwite. Your parts subtotal is adjusted by +10% over D-net automatically below the first subtotal.
Labor, Mileage, Travel Time, Others Mileage if required. Mileage and labor require amounts used (Q’ty). Other is work you needed to contract out (for example radiator repair) if required and an invoice must be sent in along with the claim.
Subtotal B Sum of Labor, Mileage, Travel Time & Others fields. Calculates automatically. Do not overwrite.
Total Calculates automatically. Sum of Parts Subtotal (w/ +10%) and Subtotal B (Labor etc.). Do not overwrite.
